| | Incident Reporting: Why, When and How to Submit |
|
CPLF tracks and reviews all near misses, injuries (bruises, breaks, sprains, and cuts), and/or illnesses that occur in the workplace. This is done through the Incident Report form, which is used by CPLF management, CPLF staff, Supportive Rights Committee, and the OH&S Committee to assist in assessing potential risks and managing identified risks. When staff and/or a supported individual with a disability has an occurrence of a near miss, behaviour outside of baseline, an injury, or an illness in the workplace, they are to: Staff are to ensure their own safety. When staff are safe, staff are to then ensure the safety of the client. Advise their supervisor during office hours immediately and follow up with a completed Incident Report within 24 hours of the incident occurring. Any CPLF staff may submit an Incident Report. If the incident is determined to be a serious/potentially serious incident, management is to follow the CPLF Critical Incident procedures.
